Incident and Accident Investigation
Description
This one-day, competency-based course focuses on learning from potential or actual incidents and accidents to prevent future incidents and accidents. This course is based on DNV (Det Norske Veritas), CSA (Canada Standards Association) and COR (Certificate of Recognition) standards. A Systematic Cause Analysis Technique (SCAT) Chart will be provided to each student.
The following topics will be covered:
- What to investigate and why investigate
- The initial response to incidents and accidents
- Collecting evidence and information
- Analyzing the information
- Determining direct causes
- Determining the real root causes
- Positive permanent corrective actions
- Positive and constructive communication of findings
Note:
- This course qualifies for 0.5 Certificate Maintenance Points (CMPs) through the Board of Canadian Registered Safety Professionals and 1.0 CMPs through the American Board of Industrial Hygiene.
